How do I get my statement?

Statements are available upon request through the CarePaths website. If you would like a statement please let me know and I will provide you with your login ID and password.



Is there anything else that I need to know?

Insurance companies will frequently reject claims based on missing information. This occurs because the information that they need is located at the bottom of the claim and they do not see the information. In order to prevent this from occurring I recommend that you highlight or circle the information at the bottom of the statement. By bringing attention to this information you may be able to prevent your claim from being rejected.
